Position Summary

The APRN/PA is an advanced practice registered nurse who combines advanced expert clinical skills with knowledge of case management and community resources to help reduce preventable readmissions, improve access to care, reduce gaps across care continuum, and contributes to improved patient outcomes.

Responsibilities

Essential Functions

  • Identify patients on assigned unit with modifiable risks to better coordinate care among various providers.
  • Recognize deviations from normal in the physical assessment; work collaboratively to formulate appropriate treatment plans.
  • Implements care management based on laboratory results, routine diagnostics, and follow-up studies.
  • Recommend and collaborate on therapeutic measures, admission, and discharge care, in accordance with patient needs and available resources.
  • Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patient population served.
  • Demonstrates caring practices by working to provide a compassionate and therapeutic environment for patients and their families.
  • Maintains knowledge and awareness of legal/regulatory issues and patients’ rights.
  • Identifies, analyzes, and collaborates to help resolve patient care issues to improve patient care.
  • Actively participates in daily multidisciplinary rounds.
  • Performs review of detailed history and physical examinations, H&Ps, and discharge summaries documented on inpatients to ensure chart completion prior to discharge or transfers
  • Writes or dictates reports and progress notes appropriately in the EHR.
  • Collaborates with the entire multidisciplinary team to ensure continuity and quality of care.
  • Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADA, FMLA and other federal, state and local standards.
  • Maintains compliance with all Orlando Health policies and procedures.

Other Related Functions

  • Acts as a role model and resource person for all OH team members and staff.
  • Maintains knowledge and awareness of all policies and procedures.
  • Actions will be in compliance with approved standards and regulatory accrediting agencies.
  • As needed, establishes relationships with professionals of other health-related groups within the community.
  • Demonstrates effective verbal and written communication with diverse populations, including physicians, employees, patients, and families.

Qualifications

Education/Training

  • Graduate of accredited APRN or PA program

Licensure/Certification

  • APRN license with qualification as a nurse practitioner in the state of Florida.
  • After July 1, 2006, licensure applicants are required to have national advanced certification from an approved nursing specialty board.
  • Maintains current BLS verification and ACLS if required for population.

Experience

  • Relevant clinical nursing experience for the population served.
